A newly released study from Coastal Fertility Specialists exposes a critical operational blind spot in IVF laboratories: mature oocyte forecasting. Based on a head-to-head comparison of 177 IVF cycles, the study found that board-certified reproductive endocrinologists (REIs) underpredicted mature oocyte yield by 4.85%, while an AI tool—Cycle Clarity—was off by just 0.71%.

Lab Efficiency Undermined by Inconsistent Daily Workload Estimates
At the day-to-day laboratory level, the Cycle Clarity algorithm delivered predictions within a 10-oocyte margin of actual counts on 76% of lab days. For fertilized embryos (2PNs), the AI achieved that margin 82% of the time. These findings highlight the operational costs of forecasting errors—misaligned expectations that can disrupt lab workflows and erode efficiency.
“REIs are excellent at identifying qualitative aspects of a patient’s cycle, but we consistently saw underestimation in total mature oocyte yield,” said Dr. John Schnorr, founding partner of Coastal Fertility Specialists. “Cycle Clarity provides the level of precision that embryology labs can depend on when planning daily workload.”
Current Estimates Require Multiple Sources of Data
Unlike REIs, who based their estimates on ultrasound scans along with estradiol and progesterone levels, the AI tool relied solely on two-dimensional follicular measurements and growth rates from ultrasound data collected before the trigger. No hormonal inputs or manual measurements were used.
In total, the 177 cycles involved 3,866 follicles larger than 10 mm, all assessed using Cycle Clarity’s automated ultrasound analysis. The uniform data input— free of user variability —allowed for consistent and fast interpretation, improving forecast reliability without sacrificing clinical nuance.

The performance of AI tools depends in part on the quality of the imaging hardware feeding its algorithm.
